According to interviews with members of the game's development team, Splintered Fate's characters and setting take heavy inspiration from the IDW comics continuity. This is evident in writing certain choices such as Angel being a young adult close in age to April and Casey and having the superhero persona "Nobody," as well as Leatherhead loathing his mutant form and desiring to return to his "natural" crocodile form. Even so, Splintered Fate is still very much set in its own world, written with a tone that isn't as family friendly as some versions of the turtles while also not being as dark as the comics it takes inspiration from.

At the time the game begins, the turtles are already firmly established in New York City and have much more experience under their belt than most other iterations starting out. They also have already long since met and established friendships with April, Casey, Angel, and Slash; Metalhead, Donatello's robot creation, also lives with them in their home deep in the sewers. Their enemies include the Foot Clan, led by Oroku Saki; the Punk Frogs, a gang of mutant frogs who make their home in the city's junkyard; and Baxter Stockman, the former owner of the StockGen research company who has since become the mayor of the city. Leatherhead is also an enemy during the game, but it's suggested that under normal circumstances, their relationship with him isn't actually all that antagonistic.

The game's plot is kicked off when mysterious portals appear across the city, one of which kidnaps Splinter. These portals are ultimately revealed to be the work of malevolent entity aiding the Shredder, who shadowy figure resembles the Rat King from the comics; another more benevolent entity, simply known as the Voice, aids the turtles in a bid to balance the scales of the "game" that the other party is playing, indicating that this world is home to higher powers that ordinary mortals cannot comprehend. Many of the game's powers and artifacts allude to the presence of other factions and characters as well, such as the alien Utroms, the witch Kitsune, Mammoth of the Pantheon, and possibly even the Daimyo from the 2003 animated series.
